Use this form (MV-44) if your registration plate is lost, stolen or defaced. If your registration plate is defaced, a Law Enforcement officer will have to sign the form to verify it is illegible. If you wish to have one of our Officer's sign the form, please call the station a head of time to ensure an Officer will be available.
